Housing Tenancy and Sustaining Services (HTSS)
Housing Tenancy and Sustaining Services (HTSS) help you remain safely and stably housed after you move in. We work with you to understand what you need to feel secure in your home and to succeed as a tenant.
As part of this support, we conduct a housing assessment and a tenancy and sustainability assessment to get a full view of your strengths, needs, and any challenges that might affect your housing. Together, we use this information to create a housing support plan tailored to your situation.
Services may include:
- Early support to address challenges like late rent or lease issues
- Education about tenant and landlord rights and responsibilities
- Guidance on building positive relationships with landlords and neighbors
- Help resolving disputes and preventing eviction
- Assistance with benefits and housing recertification
- Ongoing coaching on budgeting, life skills, and household management
- Regular check-ins to make sure your home stays safe and comfortable
- Support with any housing challenges that come up, including requests for accommodations
Please note: HTSS does not cover rent, room and board, or ongoing rental assistance, and we do not reimburse expenses you’ve already paid.
We’re here to help you maintain your home and feel confident every step of the way. If you have questions or need support, just reach out. We’re here for you!

Community Health Worker (CHW)

Community Health Workers (CHWs) are trusted members of your community who are trained to support you. They often share similar life experiences and provide personal, practical, and culturally respectful help. Think of CHWs as a lighter form of Enhanced Care Management (ECM), here to guide and support you on your health journey.
Here’s how a CHW can support you:
- Teach you about your health and help you set goals to feel your best
- Guide you through the health care system and connect you to the services and resources you need, like transportation or food assistance
- Work alongside your health care providers to create or support a plan that fits your needs
- Reach out and encourage you to use preventive care to stay healthy
- Help you sign up for or keep important programs that support your health
- Do simple screenings to connect you with the right care
- Offer one-on-one support and advocacy to prevent health issues or challenges
Your CHW is here to listen, guide, and help you access the care and support you need, every step of the way. If you think this could help you or someone you know, don’t hesitate to reach out and contact a CHW today!

CalAIM FAQ
What is CalAIM?
CalAIM stands for California Advancing and Innovating Medi-Cal. It's a statewide initiative aimed at transforming California's Medi-Cal program into a more coordinated, person-centered system that addresses not only medical needs but also social factors affecting health, such as housing, food, and daily support.
Whether you’re dealing with health issues, housing concerns, or just need help finding the right services, CalAIM is built to meet you where you are—and support you every step of the way.
Do these services cost anything?
No. All CalAIM services—including Enhanced Care Management (ECM), Community Health Worker (CHW) services, and Community Supports—are free for Medi-Cal members. There are no out-of-pocket costs.
How do I know if I qualify?
Eligibility depends on your health needs, your life situation, and Medi-Cal coverage. Some services are for people with complex medical or social needs, while others support housing, daily health, or well-being.
The best way to find out is to contact us for an eligibility assessment. We’ll help you understand which services you may qualify for.
How long can I get services?
There’s no specific time limit—you can receive services for as long as you need them and as long as you qualify. However, authorizations are reviewed by CalOptima Health and must be approved by them for continued support or to adjust your services to better suit your needs.
What’s the difference between ECM, CHW, and housing services?

|
CHW (Community Health Worker) Services |

People who need extra help staying healthy, managing a condition, or finding community support |
|
HTNS (Housing Transition Navigation Services) |

People experiencing or at risk of homelessness |

|
HTSS (Housing Tenancy and Sustaining Services) |

People already housed who need help staying stable |
Does housing support include rent payments?
No. CalAIM does not cover rent, room and board, or ongoing rental assistance. However, Housing Deposits may help with one-time move-in costs, such as a security deposit or utility setup, and HTNS/HTSS provide guidance before and after you move in.
Can I get reimbursed if I already paid for something myself?
No. CalAIM does not reimburse costs you’ve already paid out-of-pocket.
Do I need a doctor’s referral to get started?
No. You can refer yourself or be referred by a doctor, clinic, or community provider.
Will someone meet me where I am?
Yes. We can meet you wherever you’re most comfortable—at home, in a shelter, at your doctor’s office, or in the community—as long as it’s safe for you and the care team.
Will I lose my current Medi-Cal benefits or providers?
No. CalAIM services are an addition to your existing Medi-Cal benefits. You can still see your regular doctors and providers.
Can I stop services if I want to?
Yes. Participation in CalAIM services is voluntary. You can start, pause, or stop at any time.
How do I get started?
Just reach out to us! Whether you're seeking help with housing, health, or navigating services, we’ll work with you to determine what you qualify for and support you every step of the way.

"I didn't know how costly health care was going to be. My wife needs dialysis three times a week and I didn't know what I would do if I had to order an ambulance every week. ABRAZAR's drivers always help us get to the appointments on time and are always so polite and helpful. Your Transportation Program is a lifesaver."
