ECM Referral Form – Vietnamese
Enhanced Care Management (ECM) is a service available through Medi-Cal to support individuals who need extra assistance in managing their health and daily needs. ECM is designed for those with complex circumstances, providing personalized, hands-on care coordination for health and related services.
No matter where you are—on the street, in a shelter, at a doctor’s office, or at home—ECM meets you where you are. You will have a dedicated lead care manager (LCM) who helps connect all your health and social services, such as physical and mental health care, dental, and support services, making sure you get the right care when you need it.
ECM is for:
- People and families experiencing homelessness
- Adults, youth, and children who often visit the emergency room or hospital for issues that could have been prevented with proper care and support
- Adults living with serious mental health or substance use challenges
- Children and youth with serious emotional or mental health needs
- People transitioning back to the community after incarceration
- Adults at risk of going into long-term care facilities or those moving back into the community from nursing homes
- Children and youth with special health care needs beyond what California Children’s Services (CCS) covers
- Youth involved in the child welfare system, including foster care
ECM is here to support you wherever you are in your journey and to make navigating the health system easier. To learn more, please don’t hesitate to contact us.
