Housing Transition Navigation Services (HTNS)

Housing Transition Navigation Services (HTNS) assists individuals in finding and moving into safe, stable housing by offering personalized support throughout every step. We start by understanding your housing needs, preferences, and any challenges you face through a housing assessment. From there, we collaborate with you to develop a housing support plan tailored to your goals.

Depending on your needs, services may include:

  • Help finding and applying for housing
  • Gathering documents and completing applications
  • Connecting you to housing programs and financial assistance resources
  • Education on housing rights and working with landlords
  • Support during your move and help preparing your new home
  • Ongoing guidance to help you keep your housing long-term

Please note: HTNS does not cover rent, room and board, or ongoing rental assistance, and we do not reimburse expenses you’ve already paid.

Housing Tenancy and Sustaining Services (HTSS)

Housing Tenancy and Sustaining Services (HTSS) help you remain safely and stably housed after you move in. We work with you to understand what you need to feel secure in your home and to succeed as a tenant.

As part of this support, we conduct a housing assessment and a tenancy and sustainability assessment to get a full view of your strengths, needs, and any challenges that might affect your housing. Together, we use this information to create a housing support plan tailored to your situation.

Services may include:

  • Early support to address challenges like late rent or lease issues
  • Education about tenant and landlord rights and responsibilities
  • Guidance on building positive relationships with landlords and neighbors
  • Help resolving disputes and preventing eviction
  • Assistance with benefits and housing recertification
  • Ongoing coaching on budgeting, life skills, and household management
  • Regular check-ins to make sure your home stays safe and comfortable
  • Support with any housing challenges that come up, including requests for accommodations

HDS

Housing Deposit Services

Housing Deposits provide funding for one-time expenses to help individuals safely move into and set up a new home. Depending on your housing assessment and housing support plan, this service can cover items such as:

  • Security deposits and application fees
  • Utility set-up costs
  • Essential health and safety services (like pest removal or basic repairs)
  • Medically necessary items for your home, such as an air conditioner, hospital bed, or adaptive equipment

Please note: Housing Deposits do not cover rent, room and board, or ongoing rental assistance, and we do not reimburse expenses you’ve already paid.
