Community Health Worker (CHW)

Community Health Workers (CHWs) are trusted members of your community who are trained to support you. They often share similar life experiences and provide personal, practical, and culturally respectful help. Think of CHWs as a lighter form of Enhanced Care Management (ECM), here to guide and support you on your health journey.
Here’s how a CHW can support you:
- Teach you about your health and help you set goals to feel your best
- Guide you through the health care system and connect you to the services and resources you need, like transportation or food assistance
- Work alongside your health care providers to create or support a plan that fits your needs
- Reach out and encourage you to use preventive care to stay healthy
- Help you sign up for or keep important programs that support your health
- Do simple screenings to connect you with the right care
- Offer one-on-one support and advocacy to prevent health issues or challenges
Your CHW is here to listen, guide, and help you access the care and support you need, every step of the way. If you think this could help you or someone you know, don’t hesitate to reach out and contact a CHW today!
